Carnosine and Glutathione — Two Brain Compounds Worth Knowing About

Most of the conversation about anxiety treatment focuses on neurotransmitters — serotonin, dopamine, GABA, norepinephrine. And those conversations are important. But there is another layer of brain chemistry that receives far less attention and that turns out to be clinically significant: oxidative stress and neuroprotection.

The brain is the most metabolically active organ in the body. It runs hot — consuming roughly 20 percent of the body's total energy despite representing only about 2 percent of its weight. That high metabolic activity generates significant oxidative byproducts — free radicals that damage neurons and neural tissue when the brain's antioxidant defenses are not keeping pace.

Research has made increasingly clear that oxidative stress is not just a byproduct of mental health problems. In many cases it is a contributor to them. Anxiety, depression, bipolar disorder, and schizophrenia are all associated with measurably reduced antioxidant levels in the brain and central nervous system. Addressing oxidative stress is not supplementary to addressing mental health. For some people, it is part of the core work.

Two compounds are particularly worth understanding in this context: carnosine and glutathione. Both are naturally present in the brain. Both are available through diet and supplementation. Both have growing research support for their role in brain health and mental health outcomes.

What's Happening in the Brain

Oxidative stress occurs when the production of reactive oxygen species — free radicals — exceeds the brain's capacity to neutralize them. Neurons are particularly vulnerable because of their high metabolic rate and their long, branching structures. Accumulated oxidative damage impairs neuronal communication, disrupts neurotransmitter function, promotes neuroinflammation, and contributes to the kind of neural dysregulation that underlies anxiety and mood disorders.

Two of the brain's primary defenses against this damage are carnosine and glutathione.

Carnosine is a dipeptide — a small molecule made of two amino acids, beta-alanine and histidine — found naturally in high concentrations in the brain and skeletal muscle. It crosses the blood-brain barrier readily, which is clinically important: many compounds that show antioxidant effects in the body cannot enter the brain effectively. Carnosine can, and does its antioxidant work directly in neural tissue. It also modulates the glutamatergic system, helping keep glutamate — which is excitotoxic in excess and implicated in anxiety and panic — at safe levels.

Glutathione is often called the brain's master antioxidant — and for good reason. It is the most abundant antioxidant in the brain, present in virtually every cell, and central to the brain's capacity to neutralize oxidative damage and repair cellular injury. Studies have found significantly decreased glutathione levels in the brain tissue and cerebrospinal fluid of people with major depressive disorder, bipolar disorder, and schizophrenia compared to controls. Mood-stabilizing medications including lithium and valproate have been found to increase glutathione levels — suggesting that glutathione elevation may be one mechanism through which those medications produce benefit.

N-acetylcysteine (NAC), a direct precursor to glutathione, has shown clinical promise in several psychiatric conditions and is among the more evidence-supported nutritional supplements for anxiety and mood disorders.

This is why diet matters so much for mental health beyond the basic nutrition story. A brain running on poor nutrition — processed foods, high sugar, inadequate protein and micronutrients — is a brain with compromised antioxidant defenses. The oxidative damage accumulates. The neural dysregulation that drives anxiety and mood problems deepens.

This is why chronic stress makes mental health harder over time, not just in the moment. Cortisol, in sustained elevated levels, depletes glutathione. A nervous system under chronic stress is a nervous system whose antioxidant defenses are being progressively eroded.
